Put a water based lube on it to keep it moisturized, and call your vet. You need to get your pup seen today, and keep it moisturized until you get him there.

Call the vet.. the penis itself should not be swollen.. it swells at the bulbus glans.. it should not be bleeding, nor should the testicles be swollen.

He needs care NOW.. try to manually get it back in to help prevent further irritation.. use some KY or Vasaline.. make sure the skin of the sheath has not rolled inward and gently try to get it back in.

If you cant get it in, rush to a vet.. since things a swollen and bleeding, he should see a vet anyways.

The most common problem leading to bleeding in intact male dogs is almost
certainly prostate disease. The prostate can be palpated in dogs by rectal
palpation and if the prostate is greatly enlarged it would increase the
likelihood of prostatic problems as the cause of the observed bleeding. The
antibiotics are a good choice in this case. Prostate infections are often
poorly responsive to antibiotics and they must be used for long periods of
time in some dogs to get control of the infections --- it is not unusual to
have to use antibiotics for 6 to 8 weeks. Prostate hypertrophy in the
absence of infection also occurs and can lead to bleeding from the penis. I
think that this is probably the major reason that we see occasional
bleeding episodes in some male dogs who never really seem to be ill from
the problem. This is especially true of young male dogs who are sexually
frustrated by the presence of a female in heat.

Injuries are also not uncommon.

If it doesn't go back in within a 15 minute period or so there might be something wrong with him. Erections in intact dogs are normal, but bleeding is not! Best to call the vet for advice.

You can try to put it back in using some lube, but if you can't do so or it comes back out again go to the vet.
